CLIENT AGREEMENT
This agreement is made between the parent and the beneficiary of therapy, educational and/or related services herein referred to as the Client and the Rehabilitation and Empowerment of Adults and Children with Handicap Foundation, Inc. herein referred to as the Organization.
The Family and Client are seeking physical therapy, occupational therapy, speech and language therapy, education tutorials and/or related services from the Organization as duly referred by a physician.
In consideration of the above, the parties hereby agree to the following terms and conditions:
1. FAMILY/CLIENT’S SCOPE OF SERVICES
The Organization agrees to provide the needed services to the Client whenever possible by its volunteers, staff, interns and partners.
2. TERMS OF AGREEMENT
The term of this agreement will begin upon the Organization’s confirmation of the Client’s request for therapy, educational and/or related services and shall end once the Client has stopped receiving these services. The organization reserves the right to modify the duration of this agreement if, in its professional opinion, the Client’s and the context’s condition calls for it.
3. FEES AND PAYMENT TERMS
The services to be provided by the Organization will be free of charge unless otherwise specified.
4. CANCELLATION POLICY
The Family/Client agrees to provide the organization with a day’s notice of any cancellation of a scheduled session. If the Family/Client fails to provide such notice without valid reason, the Client may be placed by the Organization in a non-priority waitlist.
5. CONFIDENTIALITY
The Organization shall maintain the confidentiality of all information related to the Client’s therapy, including but not limited to, the Client’s personal information, medical history, treatment plan, and progress. The volunteer service providers shall likewise not disclose any such information to any third party without the Client’s written consent, except as required by law.
6. TERMINATION
Either party may terminate this agreement at any time upon giving the other party due notice.
7. JURISDICTION
This Agreement is governed by Philippine law, and the parties submit themselves to the jurisdiction of the courts of Mandaluyong City, Philippines..
In the event of a dispute arising out of or by reason of this Agreement, the Parties shall exert diligent efforts at reaching an amicable agreement. In case no amicable resolution is reached, any action or proceeding arising out of or in connection with this Agreement shall be exclusively brought before the appropriate courts.
The Volunteer agrees that in the instance of legal proceedings they will cooperate with the Organization as well as the Organization’s agents.
